CNETS Canada Medical Professional Survey
A survey for medical professionals, compatible with Continuing Medical Education credit requirements for the Canadian Association of Nuclear Medicine, available here.
If you know a physician who treats NET patients who is not attending this Vancouver May 13-16 meeting, please draw it to their attention and ask them to complete this survey for us. If they are unable to respond electronically we would appreciate it if you could please provide them with a hard copy for their response which they can then mail to the CNETS Canada at CNETS Canada, 4103-3219 Yonge St, Toronto, ON, Canada, M4N 3S1. We have a copy in pdf format here.
This information will help guide patient/physician interactions at the May 16 Sunday morning discussions and, more generally, contribute to the registry independent of the Vancouver International Conference.
CNETS Canada NET Patient Survey
CNETS Canada exists to make a difference to its members through its activities. Specifically, we seek to ensure that persons diagnosed with neuroendocrine tumours will be assured of ready access to ‘state of the art’ facilities and treatments under the guarantees afforded by the Canada Health Act wherever they happen to reside within Canada. This is not the case at this time. The CNETS leadership team intend to take the matter forward to Canada’s leaders.
But to be effective on your behalf, we need information about your level of comfort with your access to the currently most effective treatments and about the numbers of people at risk. We will incorporate your responses into an anonymous outcomes registry. This questionnaire asks for personal information, which it is your right to withhold. If you can complete and submit the survey, we will respect your privacy and take care to use the information provided in a confidential manner that will not identify you or allow others to do so.

We currently have no survey for caregivers and will remedy this in the near future.
